The New York State Diabetes Campaign is a five-year, $35 million initiative launched by the New York State Health Foundation to reverse the diabetes epidemic in the State. The Campaign has three goals:
- Improve clinical care and patient outcomes
The Campaign promotes the adoption of clinical best practices at community health centers, hospital outpatient departments, and private practices to improve the access to and quality of primary care for patients with type 2 diabetes. This work ensures that people with diabetes receive the right care, at the right time, in the right setting, and at the right cost to achieve the best health outcomes. - Mobilize communities
The Campaign supports community-based strategies to identify people with diabetes who do not know they have the disease, support people in managing their diabetes, and link people to quality care. - Promote policy
The Campaign advocates for private and public payment policies that enable providers to deliver more comprehensive, coordinated care for patients with diabetes. It also supports community-based policy strategies, such as making nutritious foods and recreation spaces more available.
My Pastor Saved My Life is a social marketing campaign sponsored by the Diabetes Campaign to engage and activate New York State faith-based leaders in the fight against diabetes. The campaign and this web site are designed to provide leaders with information, resources and support to help them improve diabetes prevention and care for members of their congregations. By increasing the number of New Yorkers with diabetes who receive comprehensive, high-quality care, we can save lives and prevent avoidable complications such as blindness, amputations, cardiovascular and kidney disease.
